The United States Food and Drug Administration, the federal agency that regulates drugs, food, and medical devices. Its Center for Devices and Radiological Health investigated the Therac-25 accidents from 1986 onward, eventually ordering a recall and forcing AECL to redesign the machine's safety architecture. The Therac case prompted the FDA to begin treating software inside medical devices as a regulated component in its own right.
