Lions Commission

The independent inquiry board convened by ESA and CNES to investigate the Ariane 5 Flight 501 failure, chaired by the French mathematician Jacques-Louis Lions. Its report, delivered six weeks after the accident on 19 July 1996, traced the loss to a specific Ada conversion in the inertial reference system and is now standard reading in software engineering and systems safety courses.
